Kokushibo has not made an official appearance in the anime. But those who have read the manga know that Kokushibo is a pale-skinned muscular man that has spiky black hair.
What makes him stand out is that his hair has red tips and is usually put up in a ponytail. He also has three sets of eyes on his face, making him a memorable character.

Who is Kokushibo, the Upper Moon 1 in Demon Slayer?
Kokushibo is one of the major antagonists of Demon Slayer. Before he became a demon, he was actually a human named Michikatsu Tsugikuni, who happened to be a demon slayer.
He is the twin brother of Yoriichi Tsugikuni, who is known to be the strongest demon slayer in history.
Very little is known about Kokushibo. He is very reserved and didn’t know much about the other demons in the group. He is punctual, humble, and a rule-follower, which are basically the same qualities that demon slayers have.
The main difference is that Kokushibo displays huge respect and loyalty towards Muzan. Most of his actions depended on helping Muzan achieve his goals.
Since he is number one, Kokushibo is the strongest and most powerful demon after the demon king. He was one of the oldest demons which gives him more than 500 years of combat experience.
He is a user of Total Concentration Breathing and is a marked individual. This gives him the ability to use the Transparent World. Muzan also gave him the most blood concentration, which explains his immense power.
Other than those, Kokushibo has extrasensory perception, flesh manipulation, and immense durability as some of his additional skills. To top it off, monstrous transformation is one of his special skills in which his whole physique changes.
