It has been two years since Shinpei left his home island of Hitogashima. While on the boat heading for the island, he has an odd dream about Ushio telling him to save Mio. He ends up landing his face on the chest of a woman with glasses, earning a slap from her.

He's back on the island to attend the funeral of his childhood friend, Kofune Ushio, who drowned while trying to save a little girl named Kobayakawa Shiori. Upon arriving at the island, Ushio's sister, Mio, was there to greet him.

Rumors were spreading during the funeral that an autopsy was performed on Ushio. Shiori is present at the funeral but has become mute since the accident due to the shock. Sou, another friend, informs Shinpei about how they found suspicious marks around Ushio's neck, indicating she might have been strangled.

The next day, Tetsu, the police officer on the island, tells the Shinpei and Mio that the Kobayakawa family has disappeared sometime within the night. Mio runs to Koba Mart while Shinpei chases after him.

Mio tells Shinpei that about a week ago, Shiori said something weird. She apparently saw an exact copy of herself, something like a doppelganger. Nezu, a hunter, tells them about the shadow sickness, something that plagued the island before.

It is said that when a person sees their shadow, they are sure to die soon after, and the shadow will take over and kill the rest of their relatives. Mio reveals to Shinpei that they had seen a glimpse of Ushio's shadow three days before she passed away.

According to Nezu, the sickness could be cleansed at the shrine. However, when Mio and Shinpei arrive at the shrine, no one is there. Mio claims to see Shiori.

They hear a gunshot and run towards the source. They see the woman from the boat, injured and bleeding. She pulls Shinpei closer, but as she's about to speak, Mio's shadow shoots her.

Mio's shadow has captured Mio, shooting her as well. However, she also shoots Shinpei. Shinpei suddenly wakes up back on the boat, in front of the woman with glasses — as if he just woke up from his dream.
