In the end, Wanda, herself, knew that the Darkhold has consumed her and to make sure that no one else would be able to access the Darkhold ever again, Wanda brings down the whole temple of Chthon, where the original spells of the book of the Damned are written on the walls.

Wanda went down with the temple but there's a hint that she may still be alive given that after it crumbled down the mountain, Wanda was able to destroy every version of the Darkhold in the whole multiverse as the version beside Strange gets destroyed by Wanda.

It must be noted that Wanda may or may not have survived the destruction of the temple of Chthon but given her powers, she may be very well alive as the darkness in her fades away along with the Darkhold's destruction.
