Moon Gang Tae (Kim Soo Hyun) is a nurse who works at several psychiatric hospitals while living with his older brother who has autism. He transfers from one hospital to another ever since his brother witnessed their mother's murder, which triggers his brother's trauma every spring.

Oh Jung Se breathes life to the character of Moon Sang Tae, Moon Gang Tae's older brother. He is an avid fan of Moon Young and has an impressive talent in drawing to which he started dreaming to be an illustrator. Because he, unfortunately, witnessed his mother's death, he started to have a terrible fear of butterflies which reminds him of the traumatic experience.

Go Moon Young is portrayed by Seo Yea Ji. She is a best-selling children's book author who has an antisocial personality disorder. Moon Young has a troubled childhood and a not-so-good relationship with her parents which prompted her to act like a sociopath. Soon, she encounters Gang Tae and further develops a romantic obsession with him, sometimes going over the top just to get his attention.

It's Okay To Not Be Okay actually ended on a satisfying yet open-ended note. All the mysteries that surround the three main characters were finally unveiled. The part where Sang Tae successfully overcame his fear of butterflies gave the viewers a huge amount of happy cry. No wonder it's as if we were together with him as he overcomes his trauma.

Moon Young was able to recover. She published her final book with Sang Tae as her illustrator before retiring from writing. Between all this, Moon Young tries to convince Sang Tae to stay and expressed her proposal to have him illustrate more of her stories. This was a probable hint that Moon Young is open to writing another book in the future.
