The First Omen follows the story of an American novitiate named Margaret Daino (played by Nell Tiger Free).

Margaret is sent to Rome to work at an orphanage before she becomes a nun. She is told by Father Brennan (played by Ralph Ineson) that one of the girls at the orphanage will be the mother of the Antichrist.

Father Brennan suspects it to be Carlita (played by Nicole Sorace), so he asks Margaret to bring her file to him.

Once Margaret takes hold of her file, she discovers that Carlita is one of 14 girls with the surname Scianna. She is the 14th Scianna child.

Margaret also discovers that the fifth baby is missing. But as she discovers, she is actually the fifth Scianna child.

When she discovers this, she is already carrying the Antichrist.

If you've been following The Omen franchise, you won't have a hard time figuring out why the family name, Scianna, is so familiar.

In fact, the first Omen film revealed that the Antichrist, Damien, was born to a woman named Maria Scianna, who was said to have died during childbirth.

When they opened her grave, however, they discovered that the remains were that of a jackal.

In The First Omen, however, we learn the truth. As it turns out, Margaret gave birth to twins: a girl and a boy.

The Cardinal initially planned to kill Margaret and the baby girl. But she manages to break free and stab Cardinal Lawrence (played by Bill Nighy).

Her conscience won't allow her to kill the baby boy. But this gives Luz (played by Maria Caballero), her roommate, a chance to stab Margaret.

Margaret, the baby girl, and the jackal were left in the vault to die as it went up in flames. Carlita, however, manages to save Margaret and her baby girl. But they left behind the jackal in the vault to die.

In another scene, Father Spiletto is seen giving Damien specific instructions to make his way to Ambassador Robert Thorn, which connects The First Omen with The Omen.

Sometime later, it is shown that Carlita, Margaret, and her baby have fully recovered and are living off the grid.

They are suddenly visited by Father Brennan, who warns them that the Church knows about the girl and will come for them. He also reveals the name of the Antichrist: Damien.
