Taiso Otome Fairy Tale tells the story of 17-year-old Shima Takahiko, the youngest son of a powerful family, who grew up without much affection. When he gets involved in an accident that kills his mother and permanently cripples his dominant arm, Takahiko's father banishes him to an isolated countryside residence.

There, Takahiko, who is suffering from depression, believes that he will die alone. To his surprise, a 14-year-old girl, Tachibana Yuzuki, appears at the door and proclaims that Takahiko's father sent her to be his future bride.

Tamako, Tamahiko's younger sister, pays a surprise visit, declaring she's decided to enjoy the remote family aesthetics. Disregarding her brother's feelings about the intrusion, she moves in. Before she even settles, Tamako, who is only 12 but extremely tall, makes fun of Yuzuki. Not long after, she taunts Tamahiko about his dead status in the family, devastating him.

Yuzu comes to the rescue, taking Tamahiko on a walk to show him a shrine she discovered. While Yuzuki tries to include Tamako in her activities, the latter remains cold and distant.

Things change during a spring storm that terrifies Tamako. Afraid to use the restroom alone, she wakes Yuzuki, but ends up wetting herself anyway when a thunderbolt scares them both. Yuzuki helps and supports her without complaint. Finally, Tamako reveals that her loveless upbringing in the Shima family has hurt her almost as much as Tamahiko.

When Tamako was a child, her father would dismiss her worries and pass her to servants. At school, the other girls bullied her about her height and attitude. When she decided to take a break from school, her father cautioned her against becoming like Tamahiko. Eventually, she fled to the country home having nowhere else to go.

Through Yuzuki's support, Tamako's mood improves, and she starts participating in household activities, surprising her brother. But, following this heartwarming episode, the promo reveals that Yuzu is hurt or sick in the next one. How will she and Tamahiko navigate this?
