A major storyline is awaiting Dr. Shaun Murphy (Freddie Highmore) in Good Doctor Season 5. Ahead of the hit ABC show's premiere on Monday, Sept. 27, spoilers, casting news, predictions, and a trailer have already been released.

What is the plot for Good Doctor Season 5 Episode 1?

One of the biggest storylines in Good Doctor Season 5 has to do with Shaun and Lea's (Paige Spara) engagement. The couple was in Guatemala when Lea proposed to Shaun, and he said yes.

When they returned home, Shaun shared the news of his engagement with his colleagues, and everyone was thrilled for him. He and Lea are also very excited about their upcoming nuptials, that is, until the latter became a tad bit too stressed with the wedding planning.

In the teaser released by ABC for Good Doctor Season 5, Lea is seen panicking while planning her wedding. She said she wanted her big day to be perfect because she had been married before, and things didn't go as planned.

At one point, Lea suddenly wakes up from a nightmare, and Shaun asks her point black if their wedding is still on. Lea tells Shaun that it will push through, and another scene in the teaser shows Lea in her stunning white wedding dress.

However, in what could be one of the biggest twists in the history of Good Doctor, Shaun panics on his wedding day and apologizes to Lea. As such, there are predictions that their wedding won't push through.

Elsewhere in Good Doctor Season 5, Episode 1, a young single mother learns she may have contracted cancer from a surprising source. And Mateo (Osvaldo Benavides) will find out if his issues in America will finally be resolved.

Will a fan-favorite return to Good Doctor Season 5?

Antonia Thomas, who played the role of Dr. Claire Bowen for four seasons, bid goodbye at the end of Good Doctor Season 4. However, this might not be the last time her fans will see her in the medical drama.

While speaking with TV Line, executive producer Shore wants Thomas to come back in any capacity. And the actress is also open to the idea of reprising her role.

"I'm not here to overpromise, but she wants to come back, we want her to come back... We will find an excuse to get her back for an episode or two here and there," he said.

Following her exit, Thomas admitted that her decision to quit Good Doctor didn't come easy.

"It was a really, really difficult decision, and leaving, I have to say, is a bittersweet thing for me because the last four years have been absolutely incredible. It has not been an easy decision for me to leave, so I absolutely would love to come back every now and then and say hi, bring Claire's skills from Guatemala and her stories. I'm very much looking forward to that," she said.

Did Freddie Highmore confirm that Shaun and Lea's wedding won't push through?

Highmore appeared in Good Morning America this week to talk about his character's future in Good Doctor Season 5. And the lead star seemingly dropped a significant clue that could mean Shaun and Lea's wedding won't push through.

"Shaun and Lea's marriage is certainly -- it might not be everything that you're expecting from the teaser... There's certainly a lot of twists and turns along the way this season," he said.

Highmore also revealed that Shaun would face a slew of challenges at the hospital. And he would also have to make some significant career decisions in the upcoming episodes of Good Doctor Season 5.

"There's much more for Shaun to be confronting at the hospital, too. There's a very difficult force of nature who's going to be thrown upon him in the hospital, and it will be a battle to prove himself once more and prove his medical ability," he added.

Good DoctorSeason 5 will introduce an internal nemesis for everyone

Good Doctor Season 5 won't only revolve around Shaun and Lea's engagement and wedding. The series will also present an internal nemesis that would affect all the characters in the series.

Shore told TV Line that Highmore was the one that came up with the idea of introducing an internal nemesis last season. And they are finally writing it out in season 5.

"This [nemesis] tests everybody, including Shaun. Of course, Shaun is going to have a different point of view on it," he said.

Will there be new characters in Good Doctor Season 5?

Good Doctor Season 5 will introduce Salen Morrison (Rachel Bay Jones). She is not a doctor at San Jose St. Bonaventure Hospital. And it's unclear if she's the internal nemesis that Shore is talking about.

"Her character is one of the most fun we've had. I just love what this actress brings, what this character bringsā€¦ She just has a great dynamic and great energy. She is just going, going, going," Shore teased.

And even though there are times when Salen appears scattered, she's focused on what she wants to do. Shore also described the character as a brilliant person who is sometimes a real challenge for other people.
