According to Neilsen Korea, the fourth episode lands an average Nationwdie rating of 12.3 percent. It is yet another minimal decrease from its third episode that garnered 13.5 percent.
Currently, the show’s all time high is at 13.8 percent for the Dr. Romantic 3 episode 2 airing.

The fourth episode delivered a slew of remarkable scenes, from Kim Sa Bu’s plan for the trauma center down to the reset couple’s loveline.
One of the highlights of Dr. Romantic 3 episode 4, is how Jang Dong Hwa pushed the limits of Kim Sa Bu.
The episode began with numerous patients being rushed to Doldam Hospital after a nearby villa burned to the ground.
One of the suspects is an old woman who was severely injured in the accident.
It was after the nurse found that the old woman was holding a lighter that she immediately notified the police about it.
In the other hand, Cha Eun Jae, who used to live at the villa, knew the alleged suspect and pointed out that she was not likely to do such a thing.
She described her as a cheerful person who has a positive outlook on life, so there’s no way that she could start the fire.
However, the staff shared that there was a witness to the crime and pointed to the unknown guy.
Sensing that something was off, Cha Eun Jae confronted the guy, and he turned out to be the one who started the fire.
Moreover, the old woman was stopping the guy, hence why she had the lighter.
After clearing this up, Jang Dong Hwa had a heated argument with Seo Woo Jin.
The rookie doctor pointed out that he should save the guy who is innocent of a crime instead of the alleged suspect.
However Seo Woo Jin made him understand that what matters most is the patient who badly needs help, regardless of who or what he or she did.
Jang Dong Hwa also had an intense confrontation with Kim Sa Bu, who didn't let him enter the emergency room.

For Teacher Kim, there are only two types of doctors, and one is the person who truly cares for the patient regardless of his or her status.

On the other hand, Dr. Cha finally chose the list of doctors who will be joining him at the trauma center.
He called in his daughter, Cha Eun Jae, Nurse Park, along with Joo Young Mi, as well as ER doctor Jung In Soo, anesthesiologist Nam Do Il and orthopedic doctor Bae Moon Jung.
As for Seo Woo Jin, he was invited by Dr. Cha to join his team, but he declined, showing his loyalty to Kim Sa Bu.
