Miko is still in the process of trying to cope with the fact that she sees multiple ghosts around everywhere she goes. Her brother notices her change in behavior and starts spying on her. In Episode 4, we also learn something new about her family.
Mieruko-chan follows Miko Yotsuya, a girl with the ability to see ghosts. Despite being terrified of them, her coping strategy is to pretend she doesn't see them.

Mieruko-chan Episode 4 Recap
Miko starts talking in her sleep, which her brother, Kyousuke, catches. At the convenience store, she buys a chestnut pudding, which is apparently a seasonal food. Hana says she remembers that Miko wasn't able to eat her pudding last year and says that the pudding has been waiting for her for a year. While paying at the cashier, she sees another ghost behind the counter.
At home, she and Kyousuke are watching a horror TV special about haunted photos. Miko gets freaked out and heads out to buy a drink at the vending machine. She drops her coin, and it rolls underneath the machine. While looking for it, she sees a ghost in the form of a tiny man. Thinking it's cute, she pretends to drop the coin again after picking it up. However, she ends up encountering a bigger ghost and is unable to pick up her money. A crow saves her life after it steals her money, and she runs after it.
Kyousuke notices that his sister has been acting weird. One of his classmates says that it's probably because she has a boyfriend. To confirm this, he secretly follows her after school. After Miko leaves the bookstore, he thinks that she was browsing a book for lovers by mistake. At home, Miko decides to take a bath. She reflects on how she's seen a lot of ghosts today, and the ones that would jump in and surprise her are scarier. She encounters a ghost in the bath and starts panicking. Kyousuke, making sure that there are no marks on his sister, enters and says they should take a bath. He immediately feels embarrassed, but Miko offers to wash his back.
At breakfast, Miko sees a giant ghost surrounding her family, who all notice her odd behavior. Before leaving for school, she grabs the chestnut pudding from the refrigerator and leaves it at her dad's funeral altar. He says he wanted to apologize for eating her pudding but never got the chance to. It is then revealed that her dad has been a ghost this whole time.
