What happened in Boruto: Naruto Next Generations Episode 189?

Garo and Kawaki face a clash where Garo teases that Kawaki had grown weak. Still, Kawaki proves that he is strong and has the upper hand while Delta and Koji watch the battle in front of them while handling the Leaf Shinobi. Garo, however, transforms becoming impenetrable, and started attacking Kawaki with blows he cannot counter. Garo gave a powerful blow leading Kawaki to lose consciousness.

Boruto, obviously affected, would want to join the battle but is warned by Konohamuru not to make any reckless decision as they would still have to face their own battle. Konohamuru blocks the beams directed to Boruto and raised the mission's level to the S-Rank and declaring that his top priority is to keep all of them safe.

Garo stared at them seemingly telling them not to intervene. Suddenly, Kawaki's Karma shines and Boruto felt the pain as well leading to his Karma emerging. Kawaki wakes up, not with his Karma, and successfully cuts through the hand of Garo. Kawaki was just aborting the attacks before. Kawaki proceeds to destroy Garo and blasts him, his pieces scattered on the ground. On the other hand, Boruto used his Karma to protect Team 7.

Koji finally declares: Kawaki reached Phase Three. Kawaki thinks Boruto, along with Team 7, belongs to the Kara organization. He said not to follow him as that would result in death. Boruto expressed his worry to Kawaki about not having enough knowledge of Karma. Koji explains that two Karmas resonate with each other.

As Kawaki's Karma wears off, he's left unconscious. Boruto's Karma faded as well but he did not lose consciousness. They take Kawaki back to the Leaf Village.
