If the MCU is short on something, it's definitely female villains. We already have Ghost and Hela, but artist BossLogic seems to think they should change Nightmare's gender for Doctor Strange in the Multiverse of Madness. We don't have anyone officially cast yet, but BL seems to think that Eva Green would be perfectly cast.

Personally, I think it would be a fun take on the character. Maybe they can even take a leaf out of Sandman and make Nightmare have multiple forms in the movie. Maybe she could be Green in one scene, then Mads Mikkelsen in another. With Mikkelsen being underutilized in the first movie, I guess I just want to see him back with some more meat on his performance.
Then again, some fans are thinking that Scarlet Witch may be the unwitting "villain" of the film. Some suspect that this film will have something to do with her reality-altering powers, so maybe Strange has to save her from herself. With Scarlet Witch basically removing all mutants in the House of M storyline, some are guessing that the MCU will shake it up and make her the mother of mutants instead.
It's just a theory, but I think it would be an interesting twist.
For now, you can catch both Doctor Strange and Scarlet Witch in Avengers: Endgame which is now available on digital. Doctor Strange in the Multiverse of Madness comes out May 7, 2021.
